Olaparib is a poly(ADP-ribose) polymerase (PARP) inhibitor, the first in its class, whose mechanism of action is based on the principle of synthetic lethality in cells with defective homologous recombination (e.g., in BRCA1/2 mutations). The drug is used for the treatment of various oncological diseases, including ovarian, breast (including in the adjuvant setting), pancreatic, and prostate cancers. Olaparib remains at the forefront of active research aimed at overcoming acquired resistance mechanisms (such as secondary BRCA mutations or activation of alternative DNA repair pathways) and expanding indications to other tumor types. The drug is primarily metabolized via the CYP3A4 isoenzyme, which necessitates consideration of potential drug interactions.
